AI in Action: Improving End of Life Care Planning with a Supervised Machine Learning Mortality Model.
As Artificial Intelligence (AI) continues to gain traction in healthcare, institutions face increasing pressure to adopt meaningful solutions that enhance clinical practice and patient outcomes. This case study highlights how an NCI-designated Cancer Center leveraged a supervised machine learning model to predict 90-day mortality among patients with metastatic solid tumors as part of a national Improving Goal Concordant Care Initiative. The AI model, which outperformed medical oncologists in mortality prediction, was integrated into redesigned clinical workflows and the EHR to identify high-priority patients for goals of care (GOC) discussions. The implementation led to a significant increase in structured GOC documentation and advanced care planning (ACP) billing, demonstrating the transformative potential of AI in supporting value-based, patient-centered oncology care.
