"Clinical Trials Medical Record"

Next Generation CMTS and the EMR National Working Group

The future of the Clinical Trials Management System (CTMS) lies in its seamless integration with the Electronic Medical Record (EMR), creating a unified and intelligent ecosystem that bridges clinical care, research operations, and real-world data generation. Next-generation CTMS platforms will evolve beyond traditional study tracking and financial management tools into AI-enabled, interoperable infrastructures capable of automating patient identification and trial matching, streamlining eligibility screening, supporting electronic consent and remote monitoring, and embedding research workflows directly into routine clinical care. Through standardized interoperability frameworks and bi-directional data exchange, CTMS and EMR systems will enable real-time access to structured and unstructured clinical data, reducing administrative burden, minimizing duplicate documentation, and accelerating study activation and accrual.

In support of this transformation, 2026 marks the launch of the CTMR (Clinical Trials Medical Record) National Working Group, a Phased collaborative initiative bringing together leaders from NCI-Designated Cancer Centers, community oncology programs, healthcare systems, industry, and technology partners to help define the future architecture of integrated research and clinical data ecosystems.

The CTMR Working Group will focus on advancing standards, governance frameworks, interoperability strategies, AI-enabled workflows, and operational best practices needed to fully integrate clinical trials into the longitudinal patient medical record. This effort is expected to play a critical role in supporting decentralized and hybrid clinical trial models, enhancing regulatory compliance and reporting, reducing operational silos, and creating a continuously learning health system where research and patient care dynamically inform one another.

Ultimately, the convergence of CTMS and EMR platforms guided by national collaborations such as the CTMR Working Group will serve as a foundational pillar for precision oncology, AI-driven clinical research innovation, and the modernization of cancer care delivery.
